Innovative Rx Pharmacy is a licensed non-sterile compounding pharmacy located in Indiana. We prepare customized prescription medications pursuant to valid prescriptions issued by licensed healthcare providers. Prescriptions are dispensed and shipped only to states in which the pharmacy is properly licensed and authorized to operate.

Compounded medications are prepared specifically for individual patients based on a healthcare provider’s prescription. Compounded medications may differ from commercially available, FDA-approved drug products in strength, dosage form, appearance, stability, or formulation.

Compounded medications are not reviewed or approved by the U.S. Food and Drug Administration (FDA). Your prescribing provider has determined that a compounded preparation is appropriate based on your individual clinical needs.

Innovative Rx Pharmacy does not provide medical advice, diagnosis, or treatment recommendations. All medical decisions, including prescribing decisions, are made solely by your licensed healthcare provider.

The pharmacy does not guarantee therapeutic outcomes, effectiveness, or results for any medication, including compounded medications.

Pharmacy laws, regulations, counseling requirements, and patient rights vary by state. Innovative Rx Pharmacy dispenses and ships prescriptions only to jurisdictions where it is properly licensed and in compliance with applicable state and federal laws.

Some medications dispensed by Innovative Rx Pharmacy may not be provided in child-resistant or tamper-evident packaging. Patients and caregivers are responsible for the safe storage, handling, and use of all medications, including keeping medications out of reach of children, dependents, and pets.

Improper storage or handling of medications may result in injury or harm.

Delivery times are not guaranteed. Once a prescription leaves the pharmacy’s control, Innovative Rx Pharmacy is not responsible for delays, loss, damage, theft, or degradation due to shipping carriers, weather conditions, temperature exposure, or other circumstances beyond the pharmacy’s control.

Certain medications may be sensitive to heat, cold, light, or time in transit.

Innovative Rx Pharmacy may communicate with patients by phone, email, or text message regarding prescription status, refill reminders, and pharmacy-related matters. Electronic communications may not be encrypted and may carry inherent privacy risks.

Patients may opt out of non-required communications at any time.

Innovative Rx Pharmacy complies with the Health Insurance Portability and Accountability Act (HIPAA) and applicable state privacy laws. Protected health information is used and disclosed only as permitted by law for treatment, payment, and healthcare operations.

Patients may request access to the pharmacy’s Notice of Privacy Practices by contacting the pharmacy directly.

These disclosures are governed by applicable federal law and, where not preempted, the laws of the state in which the prescription is dispensed.
